Lyon-Martin Health Services is a non-profit health clinic in San Francisco where we provide primary and preventive healthcare services to almost 2500 patients annually. Currently, 29% of our patients are people of color; 17% are transgender and 42% self-identify as queer; 84% live below 200% of the federal poverty level and 17% are homeless. We are looking for a dynamic member of one of the many communities that we serve to step up and join our team. Comprehensive Patient Support Associate

Position Summary:
The Comprehensive Patient Support Associate (CPSA) will perform a variety of tasks within the clinic to provide patients a cohesive and supportive administrative environment. They will work closely with the provider, patient care coordinator and billing teams to ensure that patients feel supported in accessing their care. They are expected to provide quality customer service in a sensitive and non-judgmental manner. They maintain a high degree of professional competence and the highest possible ethical standards while functioning as part of the care-giving team. They also have a great sense of humor, are flexible, and are able to work well with a variety of people in a quickly changing organization. The CPSA is expected to uphold and imbue in others a commitment to addressing racism, and gender and sexual discrimination within the organization and our clinical practice.
Responsibilities include, but are not limited to:
- Working with the medical and mental health teams to ensure continuity of primary care and provide a supportive and structured environment for patients and clients
- New patient registration, orientation, and education
- Enrolling patients in public benefit programs such as Healthy San Francisco and FPACT
- Managing provider schedules via patient management software
- Answering phones and directing callers appropriately
- Working with external organizations, such as pharmacies, to investigate reasons for barriers to care
- Medical record management, including releasing information as appropriate
- Providing high quality customer service and care in a sensitive non-judgmental manner
Education and Skills
- Dedication to working in a social justice orientated health care setting
- Experience maintaining professional boundaries and deescalating crisis
- Written and verbal literacy in English required.
- Fluency in Spanish required
- Prior medical office or community clinic experience
- Excellent communication, interpersonal skills and problem-solving abilities
- Ability to work cooperatively with others and demonstrate good judgment in unusual and emergent situations; also able to work independently and manage multiple tasks simultaneously.
- Computer literacy and working knowledge of patient management software (MISYS Tiger preferred)
- Knowledge of HIPAA administrative policies preferred
- Knowledge of Bay Area community resources preferred
- Experience utilizing harm reduction while working with homeless, mentally ill, and substance using patients
- Experience and dedication to working with populations with varying lifestyles, ages, sexual orientations, ethnic and cultural backgrounds, and economic status
- High School diploma or equivalent; College degree preferred
- Current CAA certification preferred
Work Schedule and Salary:
The work schedule is Monday - Friday, full-time, with some evening hours required. Salary is commensurate with experience.

MEDICAL SOCIAL WORKER/LCSW

Position Summary:
The Medical Social Worker is a Licensed Clinical Social Worker who acts as a clinical leader by providing quality mental health and case management interventions to Lyon-Martin patients receiving primary care in an integrated model. Emphasis will be placed on patients with at least one chronic illness and/or mental health issue compounded by psychosocial stressors, those who are HIV+ and/or homeless, and those at risk for becoming HIV+ and/or homeless. The ideal candidate helps to improve the health and well-being of our patients, promotes team functioning and upholds and imbues in others a commitment to addressing racism, and gender and sexual discrimination within the organization and our clinical practice.
Work Schedule/Salary
The work schedule is Monday - Friday, 40 hours per week, with some evening hours required. Salary commensurate with experience.
Education/Experience:
- Licensed Clinical Social Worker (LCSW) with valid California State License and Registration
(we cannot currently consider unlicensed candidates or MFTs)
- Completion of clinical supervision training preferred
- Experience working in a medical setting preferred.
- Experience working with homeless, mentally ill, HIV+ and substance using patients.
- Experience working with populations with varying ethnic and cultural backgrounds and economic status.
- Experience working with lesbian, bisexual and transgender/gender-variant patients.
Personal Characteristics, Skills and Abilities:
- Fluent in Spanish
- Ability to work in a small, fast-paced non-profit clinic
